Summer Camp registration is open, for Grades 6-12 (grade completed by June 2023).
ALL CAMPUSES

We’ll travel on 55-passenger buses to Hardin, Kentucky to Jonathan Creek Camps for four days and three nights of fun, worship, studying God’s Word, and more! Our lodging will be in rooms that are separated by gender and grade.

10% off registration cost (discount does NOT apply to FIRST registered student, but each ADDITIONAL student that you register).

*Please note: the deposit of $75 is due at the time of registration to secure a spot. Also, you will be able to make multiple credit card or debit card payments toward your student’s summer camp trip. After registering, you can log in to your Brentwood.church account, click on the My Invoices/Purchase History, then click on the Invoice for Summer Camp to make a payment.

LEADER COST: $100

Cost includes lodging, transportation, all meals at camp, a camp t-shirt, and activities at camp.

PARENT MEETING

Monday, June 19 |  5:30-6:30 PM | Hudson Hall | Brentwood Campus

We’ll walk through the camp schedule, POI options (formerly known as track times), Bible Study, and collect all final payment and required documents at this meeting. Packing List & Guidelines will be available by Friday, June 9.

REQUIRED FORMS

Please complete and turn in the following forms to the Student Ministry office no later than the parent meeting on Monday, June 19.

REQUIRED FOR EACH STUDENT or LEADER:

  • 2023 DIGITAL Student Release Form (*if you have not been on a student ministry trip in 2023, you must submit a new release form for the calendar year)
    –or–
  • 2023 Adult Release Form  (*if you have not been on a student ministry trip in 2023, you must submit a new release form for the calendar year)

-AND-

  • A copy of your insurance card (this can be emailed to Anna Brackins-Gilmore at [email protected]).
  • J.Creek Digital Waiver (see instructions below):
    • To Register for an account:
          • Click “Register for an account”
          • Once you register, it will say “Registration Successful” and will ask you to check your email to confirm.
          • Go to email and confirm your email.
          • After you confirm your email, it will say “login now”, but do not log in from this link. You need to go back to the original JCreek website.
            • If it asks for a code, it is p94wd75
          • Click HERE to go back to the original JCreek website.
          • Click “Log in” and input your account credentials
          • See steps below to complete JCreek form
    • To complete the digital JCreek Waiver (if you have not registered for an account, see steps above):
            • After you log in, it will say “Great, we found your event! Just a few more steps…”
              • Click “Next steps” button at the bottom
              • Click “Add a new participant”
              • Complete all steps until you see the message “Registration Completed”

OTHER FORMS:

  • 2023 Medication Form – if your student has medication. If needed, please print and turn in to the nurse at check-in on June 23.
  • 2023 EPIPEN Form – if your student has an EPIPEN is this form required. If needed, please print and turn in to the nurse at check-in on June 23.
  • 2023  DIGITAL Time Away – if your student will not be taking church provided transportation, and/or will be arriving late or leaving early from Chicago. *This form is due to student ministry office no later than Monday, June 19th*

We will be collecting the final payment + digital paperwork on the following dates at the Brentwood Campus:

  • Monday, June 19 | 5:30 PM – 6:30 PM | Hudson Hall (Parent Meeting) 

**Digital paperwork + payments can be submitted at any time to the Student Ministry office before June 19th**

CANCELLATION/REFUND POLICY

If you find you need to cancel, please notify Anna Brackins-Gilmore in the Student Ministry as soon as possible at 615-324-6126 or [email protected]. Registration closes on June 16th. Before Friday, June 16, you will receive a full refund. After June 16, the deposit is non-refundable (except in circumstances such as the death of a family member or physical illness). This is due to trip costs already incurred.
